Overview of problems and solutions formulated by RCA
When you select the events in your cause-effect model that lead to the undesirable event that you entered initially, these events are automatically formulated into problem statements. For information about selecting events, see Identifying core problems.
To view the list of problems for a cause-effect model, click Manage Problems and Solutions below the model. On the Manage Problems and Solutions panel of the root cause analysis page, you solve and manage problems and solutions.
When you click Manage Problems and Solutions, the Problems and Solutions table displays the following:
- Automatically generated problems for cause-effect models
- Any solutions that were already found for specific problems
You can add more solutions to problems by doing either of the following:
- Running the solving tool (in this case, Goldfire's Researcher)
- Adding your own solution
Short and long problem statements
Each core problem that is automatically formulated based on the selected event consists of a short and a long problem statement.
The short problem statement is:
How to prevent <event description>?
where event description corresponds to the event that you selected in the causal chain. This formulation of the problem is used as a query in automatic searches when you enter the Solve the Problem step of RCA. You can edit your query as needed.
The long problem statement is preceded by an introductory statement:
In order to eliminate the following undesirable events: <list of all undesirable events that you initially entered>, eliminate the event <event description>.
